2. Previous school information/report on child if any
3. Immunization Records of child
4. Birth Certificate of child
5. Two passport-sized photo of child

6. One passport-sized photo and original ID of authorized pick-up person(s)
7. Children may be assessed or interviewed to determine their current level of appreciation on key early childhood development indicators. The results will only be used as a guide to offering the best learning support for the child.
